  • High cost of malpractice insurance—the insurance that protects medical professionals against lawsuits—in the United States.
  • Hospital stays cost far less in many overseas countries compared to the United States. In other words, quality care, hospital meals, and rehabilitation are far more affordable abroad for many people.

For someone who doesn't have insurance , or someone having a procedure that is not covered by insurance , the difference can be enormous.

Many immigrants prefer to have treatments and procedures done in their country of origin—a sensible decision, considering just how much language barriers alone can affect the quality of their care.

Furthermore, at least 25% of immigrants and noncitizen residents in the United States are uninsured, compared to 9% of American citizens. Children with at least one noncitizen parent are also more likely to be uninsured.

Practicalities aside, many people choose to have their procedure done in their country of origin simply because it allows them to be close to family, friends, and caretakers who can assist them through their recovery .

Insurance Incentives

Some insurance companies have started promoting medical tourism. The reason behind this is simple: savings for the insured means savings for the insurance provider and vice versa.

Several insurance providers, including Aetna have programs specifically geared at promoting safe medical tourism. Some insurance providers even offer financial incentives for medical tourism, like discounts on medical bills .

That said, many insurance companies will not pay for surgery performed outside of the country unless it is an emergency.

Medical tourism is a lucrative business for many countries, and much of the money brought in by medical tourists is reinvested into the local economy and health infrastructure.

The effect of this is apparent in the spa-like luxury that some foreign hospitals offer, providing medical tourists the opportunity to be pampered during their stay for a fraction of the cost they would pay at home.

Some facilities offer hospital rooms that are more like a hotel suite than a traditional hospital room. Other hospitals offer one-on-one private nursing care, which is far more generous and attentive than the staffing ratios that most hospitals allow.

Medical tourists who seek that added layer of privacy can find it abroad. Many can return home from their "vacation" without anyone knowing they had a procedure at all.

Vacation in a Foreign Country

Medical tourists often take advantage of their stay in a foreign country to travel for pleasure by scheduling a vacation before or after their procedure.

This is an especially inexpensive way to travel to a foreign country, especially if their insurance provider is paying for the flight and the cost of staying is low. 

While it seems logical to recover on a beach or in a chalet by the mountains, keep in mind that it's important not to jeopardize your recovery.

Swimming isn't recommended until your incisions are completely closed. You may not feel up to doing much more than napping in the days following your procedure, either.

Don't let your vacation disrupt your recovery. Any time you have a procedure done, especially a surgery, it's important to listen to your body, take your medications as directed, and follow your doctor's recommendations closely.

Bypassing Rules and Regulations

Some travelers seek surgery abroad to bypass rules that are set in place by their own government, insurance company, or hospital. These rules are typically in place to protect the patient from harm, so getting around them isn't always the best idea.

For example, a patient may be told that their weight is too low to qualify for weight loss surgery . A surgeon in a foreign country may have a different standard for who qualifies for weight loss surgery, so the patient may qualify overseas for the procedure they want.

It is often surprising to many medical tourists that their physician was trained in the United States. Not all physicians are, of course, but a surprisingly high percentage of them working in surgery abroad are trained in English-speaking medical schools and residency programs and then return to their home country. The financial and practical benefits of medical tourism are well known, and you may even know someone who had a great experience. Nonetheless, the downsides of medical tourism can be just as great if not greater. Sometimes, they can even be deadly.

If you are considering a trip abroad for your procedure, you should know that medical tourism isn't entirely without obstacle and risks. These include:

Poorly Trained Surgeons

In any country—the United States included—there will be good surgeons and bad. And just as there are great surgeons abroad, there are also some surgeons who are less talented, less trained, and less experienced.

Regardless of what procedure you are getting or where, you should always do some preliminary research into the surgeon or physician who will be treating you as well as the hospital you will be treated at.

In the United States, it is fairly easy to obtain information about malpractice lawsuits , sanctions by medical boards, and other disciplinary actions against a physician.

Performing this research from afar can be challenging, especially if you don't speak the local language. Yet countless people take the risk anyway, without knowing whether the physicians who will treat them are reputable.

A physician should be trained in the specific area of medicine that is appropriate for your procedure. For example, you should not be having plastic surgery from a surgeon who was trained to be a heart doctor. It isn’t good enough to be a physician, the physician must be trained in the specialty .

A well-trained nurse can identify a potential problem and fix it before it truly becomes an issue. A poorly trained nurse may not identify a problem until it is too late. The quality of the nursing staff will have a direct impact on your care.

Once again, it's important to research the hospital staff where you will be having your procedure done. Read the reviews but don't trust them blindly. If you can, seek out a recommendation from someone who can vouch for the medical staff where you will be going.

Quality of the Facility

While researching healthcare facilities for your procedure, you want to learn not just about the quality of the facilities themselves, but about the country's healthcare system as a whole.

In some countries, there is a marked distinction between public hospitals and private hospitals. In Turkey, for example, private hospitals are considered on-par with hospitals in the states, while many locals will advise you to steer clear of public hospitals if you can.

Once you have a few options for potential facilities, you can start to investigate specifics. For one, you should find as many pictures and reviews of the facility as you can. Ask yourself whether the facility is state of the art or whether it seems dirty and outdated.

You will also need to find out if the facility has ICU level care available, in case something goes wrong. If not, there should be a major hospital nearby so that you can be transferred quickly.

To learn more about a healthcare facility, consider joining expat groups on social media for the city or country you will be traveling to. Ask the group for recommendations, or inquire about any positive or negative experiences they may have had at a particular facility.

Flying Home After Surgery

Any surgery comes with risks, including infection and blood clots . Flying home increases the risk of blood clots, especially on long-haul flights that are longer than four hours.

Try to avoid flying home in the days immediately after surgery; waiting a week will decrease the chances of developing a blood clot or another serious complication during the flight.

For longer flights, plan on getting up and walking up and down the aisles each hour to improve blood flow in your legs. You might also benefit from wearing compression socks with your doctor's approval.

If you are taking blood thinners or are at-risk of blood clots , be sure to talk to your doctor about how you can reduce your risk of blood clots after your procedure and while traveling.

Furthermore, you should know the symptoms of blood clots and stay alert.

Unplanned Illness

Any time you travel abroad, you run the risk of catching an illness that you have never been exposed to or that your body is not prepared to fight off. This is especially a concern when spending time in a foreign hospital.

If you have a sensitive stomach, you may also want to think long and hard about having surgery abroad. The food is often very different in foreign hospitals, and in some areas, there is a risk that even the water will be upsetting to your body.

Having diarrhea or postoperative nausea and vomiting makes for a miserable recovery experience, especially if you do not have a friend or family member nearby who can help you through it.

Before you travel abroad, check with your doctor to see if you need any vaccines to travel to your destination or if there are any foreign illnesses you should be aware of. Picking up an illness abroad, particularly after your surgery, can potentially be life-threatening.

Language Barriers

If you are having surgery in a country where English is not the primary language, you will need to make preparations in order to be able to communicate with the staff.

You may be pleasantly surprised to learn that the staff speaks your primary language well. If not, then you will need to consider how you will make your wishes and needs known to the surgeon, the staff, and others you will meet.

Whether you are at home or abroad, remember to speak up and advocate for yourself to make sure your needs are met. If you don't speak the local language, download a language translation app on your smartphone and don't hesitate to use it to communicate your needs. Hiring a translator is another option.

A Word About Transplant Tourism

Transplant tourism is one area of medical tourism that is strongly discouraged by organ and tissue transplant professionals in multiple countries. Most international transplants are considered “black market” surgeries that are not only poor in quality, but ethically and morally wrong.

China, for example, the country that is believed to perform more international kidney transplants than any other country, is widely believed to take organs from political prisoners after their execution.

In India, living donors are often promised large sums of money for their kidney donation, only to find out they have been scammed and never receive payment. Selling an organ in India is illegal, as it is in most areas of the world, so there is little recourse for the donor.

Then there is the final outcome: how well the organ works after the surgery is complete. With black market transplants, less care is often taken with matching the donor and recipient, which leads to high levels of rejection and a greater risk of death. Furthermore, the new organ may not have been screened for diseases such as cytomegalovirus , tuberculosis , hepatitis B , and hepatitis C . It is often the new disease that leads to death, rather than the organ rejection itself.

Finally, transplant surgeons are often reluctant to care for a patient who intentionally circumvented the donor process in the United States and received their transplant from an unknown physician.

It is important to arrange your follow-up care prior to leaving your home country.

Many physicians and surgeons are hesitant to take care of a patient who received care outside the country, as they are often unfamiliar with medical tourism and have concerns about the quality of care overseas.

Arranging for follow-up care before you leave will make it easier to transition to care at home without the stress of trying to find a physician after surgery .

Just be sure to inform your follow-up care physician where you are having your procedure done. After you return, they will also want to know what prescription medications you were given, if any.

I imagine many Americans know someone with a stockpile of antibiotics they bought during their last trip to Mexico. (Or is that just me?)

According to the CDC , the most common procedures that Americans seek out are cosmetic surgery, dental procedures, fertility treatments, organ transplants, and cancer treatment.

From pharmacies in Tijuana to luxury hospitals in Bangkok , millions of Americans travel each year for reliable care.

But which countries offer the best in cutting-edge medical tourism?

Few places have a reputation as sterling as Singapore . This nation-state is a world-class zone for finance, technology, economics, and even healthcare.

Saudi Arabia: Hajj & Umrah Pilgrimages

Hajj and Umrah are religious pilgrimages to Mecca, Saudi Arabia. Islamic religious doctrine dictates that every able-bodied adult Muslim who can afford to do so is obligated to make Hajj at least once in their lifetime. Hajj takes place from the 8th through the 12th day of the last month of the Islamic year (Dhul Hijjah). The timing of Hajj is based on the Islamic lunar calendar; its dates shift relative to the Gregorian calendar, occurring ≈11 days earlier each successive year. In 2021, for example, Hajj took place from July 17–22, but in 2022, Hajj occurred from July 7–12. Muslims can perform Umrah, the “minor pilgrimage,” any time of the year; unlike Hajj, Umrah is not compulsory.

Normally, ≈2–3 million Muslims from >183 countries perform Hajj each year, and the Kingdom of Saudi Arabia (KSA) continues its efforts to allow an even greater number of pilgrims (hajjis) attend. In a typical year, >11,000 pilgrims travel from the United States. Due to the coronavirus disease 2019 (COVID-19) pandemic, however, only 1,000 pilgrims received permission to perform Hajj in 2020. In 2021, 60,000 were allowed, and in 2022, 1 million pilgrims made the pilgrimage. In both 2020 and 2021, because no cross-border entry into the country was permitted, KSA limited Hajj pilgrims to residents of Saudi Arabia.

Performing the Pilgrimage

Most international pilgrims fly into Jeddah or Medina and take a bus to Mecca. Although the actual pilgrimage lasts only 5 days, most foreign pilgrims visit Saudi Arabia for 2–7 weeks.

On the first day of Hajj (8th day of Dhul Hijjah), hajjis travel by foot or by bus ≈5.5 miles (9 km) to Mina, the largest temporary city in the world, where most stay in air-conditioned tents.

At dawn on the 9th day of Dhul Hijjah, hajjis begin an ≈7.75-mile (12.5-km) trip by foot, shuttle bus, or train to the Plain of Arafat ( Map 10-03 [all distances shown are approximate]). During the summer months, daytime temperatures can reach 122°F (50°C). The walking route features mist sprinklers, but the risk for heat-related illnesses is high, and ambulances and medical stations are positioned along the way to provide medical assistance.

Hajj climaxes on the Plain of Arafat, a few miles east of Mecca. Pilgrims spend the day in supplication, praying and reading the Quran. Being on Arafat on the 9th of Dhul Hijjah, even for only a few moments, is an absolute rite of Hajj. Any hajji who fails to reach the Plain of Arafat on that day must repeat their pilgrimage. After sunset, pilgrims begin the ≈6.5-mile (10.5-km) journey to Muzdalifah, where most sleep in the open air. Potential health threats in Muzdalifah include breathing the thick dust and inadequate or overcrowded washing and sanitation facilities.

At sunrise on the 10th day of Dhul Hijjah, pilgrims collect small pebbles to carry to Jamaraat, the site of multiple deadly crowd crush disasters. At Jamaraat, hajjis throw 7 tiny pebbles at the largest of 3 white pillars—the stoning of the effigy of the Devil. Afterwards, pilgrims traditionally sacrifice an animal. Some purchase vouchers to have licensed abattoirs perform this ritual on their behalf, thereby limiting potential exposure to zoonotic diseases. Other pilgrims visit farms where they sacrifice an animal themselves or have it done by an appointed representative.

The next morning, on the 11th day of Dhul Hijjah, hajjis go to the Grand Mosque, which houses the Ka’aba (“The Cube”), and which Muslims consider the house of God. Pilgrims perform tawaf, 7 complete counterclockwise circuits around the Ka’aba. Because each floor of the 3-level mosque can hold 750,000 people, performing tawaf can take hours. In addition to tawaf, pilgrims have the option of performing sa’i, walking (sometimes running) 7 times between the hills of Safa and Marwah, then drinking water from the Well of Zamzam. Hajjis can travel between Safa and Marwah via air-conditioned tunnels, which have separate sections for walkers and disabled pilgrims. At the end of the day, pilgrims return to Mina (via Jamaraat) pelting all 3 pillars with pebbles.

The next day, the 12th day of Dhul Hijjah, pilgrims pelt all 3 pillars in Mina with pebbles again and then, after performing a final tawaf, some leave Mecca, ending their Hajj. Other pilgrims stay an additional night, pelt the 3 pillars with pebbles once more the next day, perform their final tawaf, and end the pilgrimage. Although not required, some hajjis include a trip to Medina, where they visit the Mosque of the Prophet, home to the tomb of Mohammed.

KSA can elect to restrict the entry of travelers coming from countries experiencing infectious disease outbreaks. In 2012, for example, KSA did not permit anyone from Uganda to attend Hajj due to an Ebola outbreak in that country; the same restriction applied to Guinea, Liberia, and Sierra Leone in 2014 and 2015.

Required Vaccines

Current Hajj vaccination requirements are available from the Embassy of the Kingdom of Saudi Arabia in the United States. As part of the Hajj and Umrah visa application process, KSA requires proof of vaccination against COVID-19 and meningococcal disease (for all pilgrims), polio (for pilgrims coming from countries where the disease is reported), and yellow fever (for all pilgrims arriving from yellow fever–endemic countries).

Coronavirus Disease 2019

In 2020 and 2021, KSA only permitted Saudi residents <65 years old to apply for pilgrimage permits. In 2022, the Saudi government reopened Hajj to pilgrims (<65 years old) from countries outside KSA. Priority was granted to those who had not previously performed the pilgrimage. For the 2020 Hajj, because COVID-19 vaccines were not yet available, KSA required Hajj pilgrims to have a negative PCR test. In 2021 and 2022, hajjis also had to provide proof of immunization with an approved COVID-19 vaccine. The Kingdom recognizes vaccines produced by Johnson & Johnson, Moderna, Oxford/Astra Zeneca, and Pfizer/BioNTech.

All travelers going to Saudi Arabia should be up to date with their COVID-19 vaccines .

Meningococcal

The Hajj has been associated with meningococcal outbreaks. In 1987, serogroup A was responsible for an outbreak and carriage by returning pilgrims to certain countries that resulted in disease among local contacts. Serogroup W was responsible for similar occurrences in 2000 and 2001.

KSA requires all pilgrims ≥1 year of age to submit a certificate of vaccination with the quadrivalent (ACYW) vaccine against meningitis, issued no more than 3 years (in the case of the polysaccharide vaccine) or 5 years (conjugate vaccine) and no less than 10 days before arrival in KSA. The conjugate vaccine is preferred because it is associated with reduced carriage, unlike the polysaccharide vaccine.

If pregnant women and children travel to the Hajj, they should receive meningococcal vaccination according to licensed indications for pregnancy and age respectively. For more details on meningococcal disease and its prevention, see Meningococcal Disease chapter.

Although KSA’s requirement for polio vaccine does not apply to adult pilgrims from the United States, ensuring full vaccination before travel is best. All pilgrims traveling from countries where polio is reported are required to show proof of vaccination ≤6 weeks prior to departure. KSA also administers a single dose of the oral polio vaccine to pilgrims coming from countries where polio has been reported, this in addition to any polio vaccine the hajji might have received in their country of origin. About 500,000 doses of polio vaccine are given at ports of entry, representing >90% of eligible pilgrims.

Bloodborne Pathogens

After completing Hajj, men shave their heads. KSA limits barber licenses and requires barbers to use only disposable, single-use blades, to limit transmission of bloodborne pathogens between customers. Remind male travelers to patronize only officially licensed barbers whose establishments are clearly marked. The Centers for Disease Control and Prevention (CDC) recommends all travelers to KSA, particularly health care workers or other caretakers participating in Hajj, be up to date with routine immunizations, including hepatitis B vaccine.

Enteric Infections & Diseases

Diarrheal disease is common during Hajj. During the pretravel consultation, inform travelers about prevention, oral rehydration strategies, proper use of antimotility agents, and self-treatment of travelers’ diarrhea (TD) with antibiotics. Most TD in hajjis is bacterial (≤83%), with smaller proportions caused by viruses and parasites. More information on TD can be found in Sec. 2, Ch. 6, Travelers’ Diarrhea .

The World Health Organization recommends that travelers visiting farms, or other areas where animals are present, practice general hygiene measures, including avoiding contact with sick animals and regular handwashing before and after touching animals. Travelers should avoid consuming raw or undercooked animal products, including milk and meat.

Respiratory Infections & Diseases

Respiratory tract infections are common during Hajj, and pneumonia is among the most common causes of hospital admission. The risk for respiratory infections underscores the need to follow recommendations from the Advisory Committee on Immunization Practices for pneumococcal conjugate and polysaccharide vaccines for pilgrims aged ≥65 years and for younger travelers with comorbidities.

Although not a requirement, the CDC strongly recommends that hajjis be fully vaccinated against seasonal influenza. Behavioral interventions, including regular handwashing with soap and water, properly wearing a facemask, cough etiquette, and, if possible, physical distancing and contact avoidance, can help mitigate the risk for respiratory illnesses among pilgrims. Assess travelers for respiratory fitness, administer necessary vaccines, and prescribe adequate supplies of portable respiratory medications (inhalers are easier to transport than nebulizers) as needed.

Crowded conditions, even outdoors (densities can reach 9 pilgrims per square meter), can increase the probability of respiratory disease transmission during Hajj, including COVID-19 and Middle East respiratory syndrome (MERS). At the time of writing, no Hajj-associated cases of COVID-19 or MERS have been reported. Many pilgrims come from areas highly endemic for tuberculosis (TB); some arrive for Hajj with active pulmonary disease. Educate pilgrims about the risk for TB, and instruct them to follow up with their doctor if they develop symptoms of active TB.

Middle East Respiratory Syndrome

MERS, caused by the Middle East respiratory syndrome coronavirus (MERS-CoV), was identified in Saudi Arabia in 2012 (see Sec. 5, Part 2, Ch. 14, Middle East Respiratory Syndrome / MERS ). Domestic cases in and around the Arabian Peninsula and exported cases, including in the United States, have ranged from mild to severe; ≈35% of reported cases have been fatal. Close contact with someone who has confirmed MERS-CoV infection, exposure to camels, and consuming raw or undercooked camel products (e.g., milk, urine, meat) are all considered risk factors for human infection with MERS-CoV.

Skin Infections

Chafing caused by long periods of standing and walking in the heat can lead to bacterial or fungal skin infections. Advise travelers to keep their skin dry, use talcum powder, and to be aware of any pain or irritation caused by garments. Travelers should disinfect open sores and blisters and keep them covered. As a sign of respect, pilgrims enter the Grand Mosque with the tops of their feet uncovered; while most hajjis perform tawaf in their bare feet, encourage travelers with diabetes to wear appropriate, protective footwear.

Vectorborne Diseases

Aedes mosquitoes, vectors for dengue, and Anopheles mosquitoes, vectors for malaria, are present in Saudi Arabia. Travelers should follow mosquito bite prevention measures outlined in Sec. 4, Ch. 6, Mosquitoes, Ticks & Other Arthropods . Dengue has been documented in Mecca and Jeddah, but not in association with Hajj. KSA conducts extensive spraying campaigns before Hajj, and especially targets the housing units of pilgrims from malaria- and dengue-endemic areas. The cities of Jeddah, Mecca, Medina, Riyadh (the capital of KSA), and Ta’if have no malaria transmission, and prophylaxis against malaria is neither recommended nor required for pilgrims.

Animal Bites

Pilgrims bitten by animals should seek immediate medical attention to address any potential rabies exposure (see Sec. 4, Ch. 7, Zoonotic Exposures: Bites, Stings, Scratches & Other Hazards , and Sec. 5, Part 2, Ch. 18, Rabies ).

Climate & Sun Exposure

Heat is a threat to the health and well-being of all travelers; both heat exhaustion and heatstroke can cause incapacitation and death among pilgrims (see Sec. 4. Ch. 2, Extremes of Temperature ). Travelers are particularly at risk when Hajj occurs during summer months; the average high temperatures during June–September are ≥110°F. High temperatures combined with high humidity can lead to a heat index indicative of an extreme heat warning. High heat alone can exacerbate chronic conditions.

Depending on the exact location of their lodgings within Mina and whether they use trains or shuttle buses to get from one location to another, hajjis might walk up to ≈35–40 miles (≈55–65 km) over the 5 days; about 45% of pilgrims walk during the Hajj rituals. Counsel pilgrims to stay well hydrated, wear sunscreen, and seek shade or use umbrellas when possible. Religious leaders have ruled that it is permissible for hajjis to perform some rituals after dark. In addition, except for a pilgrim’s required presence on Arafat on the 9th day of Dhul Hijjah, most other compulsory rituals can be postponed, done by proxy, or redeemed by paying a penalty.

Chronic Health Conditions

Hajj is arduous, even for young, healthy pilgrims. Because many Muslims wait until they are older before performing Hajj, they are more likely to have chronic health conditions. Travelers caught up in the experience of Hajj or Umrah might forget to take their usual medications. People with chronic medical conditions should have a health assessment before traveling to Hajj. Tailor a plan for each traveler’s unique risks, including adjusting the usual medical regimen if necessary, ensuring an adequate supply of medications, and providing education about symptoms that indicate a condition requiring urgent attention.

Pilgrims with diabetes should have a customized management plan that enables them to meet the arduous physical challenges of the Hajj. They should bring adequate amounts of all medications, plus syringes and needles if they are insulin dependent. They also should carry an emergency kit with them on their pilgrimage; the kit should include easily accessible carbohydrate sources, glucagon, a glucometer and test strips, urine ketone sticks to evaluate for ketoacidosis, and a list of medications and care plans. Emphasize the importance of wearing durable and protective footwear to reduce the incidence of minor foot trauma, which can lead to infections.

Menstruation

Muslim law prohibits a person who is menstruating from performing tawaf. All other rituals are independent of menses. Because pilgrims generally know well in advance that they will be making a pilgrimage, those who intend to manipulate their menstrual cycle should consult with a physician 2–3 months before the journey.

Fire is a potential risk during Hajj. In 1997, open stoves set tents on fire, and the resulting blaze killed 343 pilgrims and injured >1,500. In 2015, a hotel caught fire and >1,000 pilgrims were evacuated. KSA no longer allows pilgrims to erect their own lodgings or prepare their own food; permanent fiberglass structures have replaced formerly makeshift accommodations.

Traffic-Related Injuries

As in other countries, motor vehicle crashes are the primary safety risk for US travelers to KSA. Remind Hajj pilgrims of the importance of seatbelt use in any vehicle, including buses (see Sec. 8, Ch. 5, Road & Traffic Safety ). Encourage pilgrims to be mindful of their own safety when they walk long distances through or near dense traffic.

Trauma is a major cause of injury and death during Hajj. Hajj is associated with dense crowding, leading to crush disasters or stampedes. Thousands of pilgrims were killed during a crush at Mina in 2015, making it the deadliest Hajj disaster on record. Death usually results from asphyxiation or head trauma, and large crowds limit the movement of emergency medical services, making prompt rescue and treatment difficult.

Travelers who become ill during Hajj have access to medical facilities located in and around the holy sites. An estimated 25,000 health care workers are typically in attendance, and medical services are offered free of charge to all pilgrims. For safety reasons, KSA advises that children, the frail elderly, seriously ill, and pregnant people postpone Hajj and Umrah.

Life-threatening conditions expected in Gulf Coast as Hurricane Francine nears

What to know about hurricane francine.

  • Francine is barreling toward Texas and Louisiana, and forecasters expect life-threatening storm surges.
  • At 11 p.m. ET, the storm was about 185 miles east-northeast of the mouth of the Rio Grande and 295 miles southwest of Morgan City, Louisiana.
  • A hurricane warning is in effect across most of the Louisiana coastline, while a storm surge warning covers the area from High Island, near Houston, all the way to the Mississippi and Alabama border.

Biden declares federal emergency in Louisiana

President Joe Biden tonight declared a state of federal emergency in Louisiana ahead of landfall for Hurricane Francine tomorrow.

The declaration includes federal assistance for Louisiana's response, as well as federal coordination for disaster and relief efforts from the Federal Emergency Management Agency, the White House said in a statement.

Federal officials will help with "alleviating the hardship and suffering caused by the emergency on the local population" while also providing "appropriate assistance for required emergency measures," it said.

Federal money will be available for evacuation, shelter and mass care for the roughly 30 parishes in the state that could be affected, the White House said.  

FEMA's Sandra L. Eslinger has been appointed to coordinate federal recovery operations, it said.

New Orleans public schools closed as a precaution

Public schools in New Orleans will be closed tomorrow and Thursday to keep children, teachers and staff members out of harm's way when Francine hits sometime tomorrow, NOLA Public Schools said in a statement.

Instruction ended early today as part of the response, according to the statement.

"Tuesday and Wednesday’s closures are intended to provide families adequate time to make necessary preparations and stay safe, particularly in light of potential flooding and other hazards in the aftermath of the storm," the district said.

The state of the district and the storm's impacts will be evaluated Thursday, and a decision about reopening campuses will be made then, the district said.

Francine moves closer to Louisiana's coast

The latest update from the National Hurricane Center tonight has placed Hurricane Francine just a little closer to the Louisiana coast, where impacts could be begin tomorrow, forecasters said.

The storm was 295 miles southwest of Morgan City, according to the 11 p.m. ET update. It continued to move northeast at 10 mph and produce maximum sustained winds of 75 mph, the center said.

A federal hurricane warning was in effect from Vermilion Parish to Grand Isle in Louisiana, all west of New Orleans. The same stretch of coast was also the subject of a storm surge warning.

The coastline could get rain amounts of 4 to 8 inches, with 12 inches possible, and a storm surge of 5 to 10 feet, the hurricane center said.

The storm was expected to gain some strength and accelerate before it strikes land tomorrow afternoon or evening, it said.

911 callers may get no response if Francine is overhead, officials warn

In Jefferson Parish, Louisiana, officials urged residents to be so prepared and out of harm's way they won't need to call 911 after Francine makes landfall.

Because if they do call 911, it's possible no one will be able to respond. Parish President Cynthia Lee Sheng mentioned a no-response policy during a news conference today, and the sheriff's office confirmed it: Once tropical storm-force winds — defined as maximum sustained winds of 39 mph to 73 mph — kick in, sheriff's deputies are ordered to shelter in place. (Hurricane status begins at 74 mph.)

"Deputies typically remain on the road to answer calls for service for as long as it is safe to do so," Sheriff's Capt. Jason Rivarde said by email. "That threshold is once tropical storm force winds enter the area. We use the same standard for restoring service and beginning damage assessments after the storm passes through."

It wasn't clear whether the same standard applies to fire and medical first responders. The parish abuts New Orleans to its west and south.

Rivarde said keeping deputies off the road helps the community in the long term. "It is necessary to pull them off the road to allow them to shelter-in-place as the storm passes to preserve their ability to respond following the event," he said.

Louisiana communities order curfews

Lafourche Parish, west of New Orleans, and Washington Parish, north of the city, announced that curfews will be enforced as the area gets ready for Francine.

The moves could reduce the number of people needing help as hurricane conditions arrive, and they might offer security for business operators concerned about abandoning their locations.

In Lafourche, the curfew starts at noon tomorrow and lasts through sunset Thursday, officials said in a statement. Deputies will allow a grace period to allow workers to get home, it said.

"All businesses should be closed by noon on Wednesday for employees to make it home before the strongest storm impacts begin," the city said.

Washington Parish enacted an emergency order that declares public spaces off limits from 10 p.m. tomorrow to noon Thursday.

New Orleans officials want residents to wrap up preparations

New Orleans asked residents to wrap up their storm preparations tonight.

According to the storm's forecast path, the city could be affected by the hurricane's right flank, which historically does the most damage along the Gulf Coast.

With landfall forecast for tomorrow on the Louisiana coast, possibly along the Mississippi River delta plain, officials say time for safe preparation is running out.

Residents are urged to shelter indoors overnight and throughout the storm's presence.

Francine strengthens into a hurricane

Francine is officially a hurricane, according to the National Hurricane Center's 8 p.m. update.

The center estimated the storm's sustained winds at 75 mph, which is 1 mph above the threshold for hurricane status.

"Additional strengthening is expected through Wednesday morning," the hurricane center said.

The storm has sped up slightly to 10 mph and will also continue to accelerate before landfall, it said.

"A faster northeastward motion is expected tonight and Wednesday," the hurricane center said.

Francine continues to move northeastward across the western Gulf of Mexico and is expected to make landfall in Louisiana tomorrow afternoon or evening.

Cameron, Louisiana, has been hit by four major hurricanes in the last 20 years, turning the Gulf Coast community into a ghost town . And once again, the residents who remain are packing up and evacuating as Tropical Storm Francine barrels toward the Louisiana coast.

Last night, officials ordered a mandatory evacuation for all of Cameron Parish, warning of strong winds, heavy rain and peak storm surge up to 5 feet . The storm is forecast to make landfall as a hurricane within Cameron’s radius tomorrow, threatening a community that has already suffered millions of dollars in hurricane damage. Cameron is about 70 miles southeast of Beaumont, Texas. 

Lifelong Cameron Parish resident Anna Dupont, 21, spent today packing and gathering supplies — gasoline, water, cat litter, trash bags and emergency MRE rations. Dupont and her family plan to ride out the storm from the nearby fire station, where her father works as the fire chief. They hope to provide aid to any remaining locals following landfall.

In recent decades, scientists have observed an increase in rapidly intensifying hurricanes linked to rising ocean temperatures resulting from global warming. And as Tropical Storm Francine moves into the Gulf of Mexico, record high water temperatures are fueling it. Over the last 50 years, the Gulf of Mexico, which borders Cameron, has warmed twice as fast as the rest of the world’s oceans.

DuPont says this storm is the last straw. Hurricanes have destroyed her home twice, but her family has stayed through it all. In the fall, DuPont is leaving her hometown and moving inland — and with the storm looming over her parents’ newly renovated house, she hopes they’ll do the same.

“I’m honestly over it at this point,” DuPont said. “I’ve packed my records and left everything else. This time, I just hope I can convince my parents to move.” 

Metro New Orleans under hurricane watch

Metropolitan New Orleans is under a hurricane watch, according to the National Hurricane Center.

Lake Maurepas and Lake Pontchartrain are also included in the hurricane watch. A hurricane warning is still in effect on the Louisiana coast from Cameron to Grand Isle.

Francine was still classified as a tropical storm at 5 p.m. with 65 mph maximum sustained winds. The storm is about 135 miles east of the mouth of the Rio Grande and 360 miles southwest of Morgan City, Louisiana, moving northeast at 10 mph.

Francine is expected to strengthen into a hurricane tonight before landfall in Louisiana tomorrow. It is expected to quickly weaken after making landfall.

"After landfall, the center is expected to move northward into Mississippi on Wednesday night or Thursday," the hurricane center said.

Mississippi governor declares state of emergency

Mississippi Gov. Tate Reeves declared a state of emergency this afternoon ahead of Francine's landfall.

Reeves said on X that the declaration would help the state "mobilize state assets, and respond as necessary."

Mississippi's coastline is so far forecast to avoid a direct hit, but it is likely to experience tropical storm-level effects, including possible life-threatening storm surge along the coast and waterways, according to the National Weather Service.

More evacuations for Louisiana parishes

Mandatory evacuations were issued for Lafourche and Plaquemines parishes on Louisiana's coast today.

In Lafourche, evacuations will begin at 5 p.m. They are mandatory for residents "in a FEMA or State Housing Units and South of the Leon Theriot Locks" and voluntary for those in the Lower Bayou Blue Community and Eagle Island Road and anyone in a mobile home, according to the parish website .

In Plaquemines, mandatory evacuations started at 7 a.m. in seven areas and will begin at 2 p.m. for the communities of Eastbank and Venice north to Alliance.

More than 100K sandbags deployed in south Mississippi

The Mississippi Emergency Management Agency deployed more than 100,000 sandbags in south Mississippi this morning in preparation for Francine.

The agency also shared a list of locations where residents can find sandbags.

Power outages expected in Texas; impacts already being felt in the South, officials said

Power outages are expected to affect Texas as Francine travels closer to the Gulf Coast, officials said.

Gov. Greg Abbott said at a news briefing this afternoon that he expects power providers to be prepared for potential outages, adding that CenterPoint's response to outages during Hurricane Beryl were "unacceptable."

"We know that even with the greatest preparation, winds can sometimes cause power lines to fall to the ground," Abbott said. "When that happens, we fully expect power companies to be prepared in advance to restore those power lines in a matter of hours, not in a matter of days."

Abbott said the state's response to the coming storm "has already been robust," with resources deployed to areas expected to be affected, including water rescue teams, food and water. The National Guard is also on standby, he said.

Heavy rain and storm surge have started to affect state highways in the Lower Rio Grande area in South Texas, said Marc Williams, executive director of the state Transportation Department.

Williams warned residents to be aware of their surroundings and not to drive into floodwaters.

"Also, be aware of potential for downed power lines," he said. "Having that awareness is critical, especially if you’re going to potentially be out on the road."

Baton Rouge residents collect sandbags ahead of landfall

Residents in Baton Rouge were collecting sandbags at Cadillac Street Park in preparation for Francine's landfall.

Jynita Christmas hopes the storm and any potential damage are limited.

"Hopefully, it doesn’t do too much to us," Christmas said. "I’m hopeful that is what it is, just a lot of rain and wind."

Christmas said she's concerned the rain may cause flooding.

"When it rains a little bit, the area I stay in, it kind of floods," she said.

Christmas said she'd been up since 7 a.m. making trips to the grocery store and trying to find sandbags to help prepare her home.

Tropical Storm Francine is likely to strengthen to a hurricane later today or tonight before it makes landfall in Louisiana tomorrow.

"On the forecast track, Francine is anticipated to be just offshore of the coasts of northeastern Mexico and southern Texas through this afternoon, and then move across the northwestern Gulf of Mexico, making landfall in Louisiana on Wednesday," the hurricane center said. "After landfall, the center is expected to move into Mississippi on Wednesday night or Thursday."

Francine is expected to bring 4 to 12 inches of rain and damaging winds to Louisiana. A few tornadoes are also possible tomorrow morning near the coasts of Louisiana, Mississippi, Alabama and the Florida Panhandle, the hurricane center said in an update this morning.

The storm is about 120 miles southeast of the mouth of the Rio Grande, with maximum sustained winds of 65 mph. It's moving north-northeastward at 8 mph across the Gulf of Mexico.

A hurricane warning is still in effect for the Louisiana coast from Sabine Pass east to Grand Isle. A storm surge warning has been issued from the mouth of the Mississippi River east to the Mississippi and Alabama border.

A tropical storm warning is in effect for the Mississippi coast from the mouth of the Pearl River to the Mississippi-Alabama border, according to the hurricane center.

Louisiana Gov. Jeff Landry declares state of emergency

Louisiana Gov. Jeff Landry has declared a state of emergency ahead of Francine's forecast landfall in the state tomorrow.

Francine is expected to strengthen into a hurricane before it makes landfall in the state, according to the National Hurricane Center.

The storm is expected to bring 4 to 12 inches of rain to the state, as well as damaging winds and a few potential tornadoes, tomorrow morning near the coasts of Louisiana, Mississippi, Alabama and the Florida Panhandle, the hurricane center said in an update this morning.

“This State of Emergency will allow parishes statewide to have the resources to help protect the life, safety, and welfare of the citizens of Louisiana," Landry said in a statement, adding that his administration will stay in contact with local officials and first responders.

LSU closes campus ahead of Francine's landfall

Louisiana State University announced that its campus will be closed today and classes will be held remotely tomorrow and Thursday.

The university had said it would remain open today, but it released an updated statement announcing the campus closure. All events on campus starting at 4:30 p.m. today will also be canceled through to Thursday.

Tropical Storm Francine is expected to make landfall on the Louisiana coast tomorrow as a hurricane, according to the National Hurricane Center. LSU is in Baton Rouge, near the coast, and it is forecast to get tropical storm conditions .

"LSU residence halls and on-campus apartments will remain open and food will be available to on-campus residents," LSU said in a statement. It also advised students living on campus to be ready to shelter in place and students living off campus to gather emergency supplies and consider safer shelter if needed.

More coastal communities in Louisiana ordered to evacuate

Another part of low-lying, coastal Louisiana has told its residents to leave ahead of Tropical Storm Francine, which is due to hit the state tomorrow as a Category 1 hurricane.

Terrebonne Parish, south of Baton Rouge, with a population of more than 100,000, issued an emergency declaration this morning and told people to leave the area by no later than 6 a.m. tomorrow.

The area is famous for its Cajun culture and use of Louisiana French.

This morning, Francine remains a tropical storm with sustained winds of 65 mph — but it's expected to strengthen into a hurricane as it begins to move northeast toward the Gulf Coast.

Tropical storm force conditions are expected to arrive along the Upper Texas Coast between 2 p.m. and 8 p.m. ET today and along the Louisiana coast by 2 a.m. ET tomorrow. Francine is expected to make landfall tomorrow afternoon or evening as a hurricane somewhere along the Louisiana coast. 

One update to the forecast compared to yesterday is a slight nudge to the east for possible landfall, putting Baton Rouge and New Orleans at higher risk for impacts. However, we are in the timeframe where changes in the track are expected — i.e. the windshield effect — so all locations from Lake Charles to New Orleans remain in the cross-hairs.

For southern Louisiana, rainfall amounts of up to 12 inches and wind gusts in excess of 100 mph will be possible through tomorrow across Louisiana.

Hurricane and storm surge warning area expanded along Louisiana coast

The areas covered by hurricane and storm surge warnings have been expanded by the National Hurricane Center, as Tropical Storm Francine slowly edges towards the Louisiana coastline.

A hurricane warning now stretches from Sabine Pass on the Louisiana-Texas border, all the way to Grand Isle, south of New Orleans — covering almost all of Louisiana's Gulf Coast.

The area covered by a tropical storm warning was widened to the Louisiana coast east of Grand Isle, to include the New Orleans metro area.

Potentially dangerous storm surges are expected from the mouth of the Mississippi River, near New Orleans, to the Mississippi-Alabama border, over the next 36 hours.

New Orleans urges people to prepare for Storm Francine's arrival and shelter-in-place from tomorrow

The people of New Orleans have been urged to complete any preparations for Tropical Storm Francine by tonight and to strongly consider staying at home tomorrow and Thursday. The city is under a tropical storm watch.

City leaders told a news conference last night that the storm is expected to become a category 1 hurricane and could have a severe effect on the city and wider region.

New Orleans Office of Homeland Security and Emergency Preparedness Director Collin Arnold said: "We do believe that this is a shelter-in-place scenario storm and that the public should pay a lot of attention to this."

Louisiana Gov. Jeff Landry urges people to be 'cautious and vigilant'

People in at-risk areas of Louisiana should have a plan in place and be aware of the dangers of the storm, Louisiana Gov. Jeff Landry said at a news conference last night.

"We want everyone in that state to be cautious and vigilant. We don't want to downplay this event, but we also do not want people to panic," he said.

Landry passed a state-wide emergency declaration last night and there are 23 local emergency declarations across the state, a number that could increase.

How bad will Francine be? 10ft storm surges and up to 12in of rain expected

Dangerous coastal storm surges of up to 10 feet are a major concern ahead of Francine's arrival in the U.S. The Louisiana coast from Cameron to Port Fourchon and Vermilion Bay could see between 5 and 10 feet of storm surge.

"The deepest water will occur along the immediate coast near and to the east of the landfall location, where the surge will be accompanied by large and dangerous waves," the National Hurricane Center said.

In addition, 4 to 8 inches of rain is expected from northeast Mexico to Mississippi, with 12 inches forecast in some areas. There will also be "life-threatening surf and rip current conditions," the NHC said.

Evacuations ordered for coastal areas of Louisiana and Mississippi

People living in low-lying, coastal areas of Louisiana have been strongly advised or ordered to move inland ahead of Francine's arrival on Wednesday.

Cameron Parish, a coastal community between Lafayette and Houston, where the storm is expected to make landfall, is subject to a mandatory evacuation order as of 6 a.m. today.

Grand Isle, a Louisiana town on a narrow stretch of land in the Gulf of Mexico, issued a voluntary evacuation for residents and a mandatory evacuation order for campers and RVs.

Meanwhile, Pass Christian, Mississippi, strongly advised the evacuation of Pass Christian Harbor.
