This must be completed within 24 hours before arrival in to Tanzania and once completed will issue a unique health code which should be presented upon arrival.

Here is the link 😊

BTW I will be travelling US to TZ next week so will be able to give first hand feedback on all aspects of airline travel, protocols, testing etc. including using this new form. I'll post back on here and also if you are interested in following along I'll also be blogging the trip at

Thanks Ellie. Can’t wait to follow your blog.

Is this a new requirement? Is it a likely precursor to other new requirements? (I realize that might require a crystal ball. Just wondering if people in the know have a feel for how the wind is blowing.)

So more likely, I will be at the mercy of finding a laptop hookup and wifi and printer, receiving the health code back, and printing out the results, during a 3 hour connection at a foreign airport? 😮

Ct - breezes. It seems to be just an online version of the health form that was already required to be completed before landing rather than a new requirement as such. They have just shifted the process online.

I agree that the 24 hour window is somewhat unwieldy when travel from US ..... since it really requires it to be completed while in transit somewhere ... for me DOHA. Oh well fingers crossed for internet access.

Will report back in 8 days (if you don't hear from me I'm still sitting on the tarmac somewhere waiting for my unique health code to load)😉

Won't work in practice for reasons stated above. How does one cross at a land border?

Yes when we arrived at JRO in January this was a paper form that we had to fill in and then give to the guys who were at the “Health Screening” desk where they checked our temperature and asked a few other questions before we then progressed through to the rest of the immigration/visa process. As others have said it has obviously been moved online now.

I agree. The two will run in tandem much like Visa applications. On:Line can never fully replace form filling for many reasons.

Much looking forward to your Blog!!

NoExpert - thanks for the vote of confidence - now I'm under pressure to deliver 😁

CT Breezes - yes as you guessed I am flying DFW - DOH - JRO on Qatar. As an aside - Right now Qatar are telling me no PCR test required by them since TZ does not require one ...... however I am going ahead and get one anyway before travel just to have on hand in case. It will be interesting to see if I am asked for it at any stage - will let you know on that.

Also I just checked with clients who travelled Spain to TZ arriving 26th April and they were able to just use the paper health check forms on arrival.

Got to go pack 😉

Tanzania Public Health Surveillance Form

Apply the health form for Tanzania online by clicking on the link below

Tanzania Health Form

The Tanzanian authorities introduced a short health declaration for all incoming travelers to Tanzania during the ongoing Covid-19 pandemic. The form is mandatory to be completed before arriving in Tanzania and is available entirely online.

Completing the Tanzanian Public Health Surveillance Form takes only a couple of minutes and can be done using any Internet-accessed electronic device.

Many countries worldwide introduced health declarations as an additional safety measure to tackle the coronavirus pandemic and control the spread of new cases. The health declaration then ensures the safety of both incoming visitors and Tanzanian citizens. Since travelers are required to provide some essential personal, travel, and health details, the Tanzanian officials are able to immediately contact a traveler who may have been infected or contacted an infected person.

Every applicant who successfully submits the Tanzanian Health Form will receive its confirmation to the email address provided when filling out the online form.

Who must complete the Tanzania Health Form?

The Tanzania Health Surveillance Form is mandatory for everyone arriving in Tanzania. Regardless of age, nationality, and vaccination status, every person needs to complete the health form before the planned trip.

Children traveling to Tanzania should complete a separate health form for Tanzania. Nevertheless, their legal guardians or parents need to make the application on their behalf. They also must ensure that all the data of their underage travel companion is correct and up-to-date.

When should you submit the Tanzania Health Form application?

You must submit your Tanzania Health Surveillance Form application within 24 hours before the planned arrival. It is extremely important not to complete the declaration earlier since all the data provided must be up-to-date. Filling out the online declaration before this specific time frame may result in some problematic issues upon crossing the Tanzanian borders or even denying entry.

The Tanzanian authorities introduced the requirement of a specific time to complete the form to ensure the safety of all incoming people and Tanzanian nationals.

Other requirements to enter Tanzania during Covid-19

Apart from the necessity to complete the Tanzania Health Surveillance Form, all travelers to Tanzania need to submit a negative report of the Covid-19 test taken within 96 hours before the scheduled departure. The Tanzanian authorities recognize both RT-PCR and NAATs.

Travelers who arrive from high-risk countries, where an increased number of coronavirus cases have been reported, must additionally be tested upon arrival at their own cost. If tested positive for Covid-19, they are subject to a 14-day quarantine that may be ended only upon receiving the negative test result.

If you plan to visit also Zanzibar, you should know that Zanzibar requires a separate health declaration dedicated only to the island.

Before traveling to Tanzania, please check the recent entry protocol to avoid any issues during your entire trip and stay.
