Description

Uses Gravol 50 (Dimenhydrinate) is a motion sickness treatment which is taken to prevent people who are travelling from experiencing dizziness, vomiting, nausea and other travel sickness symptoms. The main ingredient in this drug is a type of antihistamine.

Dosage and Administration Gravol 50 (Dimenhydrinate) tablets are taken by mouth. See your doctor before using this medicine, so that you can be told the correct amount to take. Adults taking it for motion sickness are sometimes told by their doctor to take 50mg to 100mg by mouth, at 4 to 6 hour intervals. However, patients are generally warned not to take more than 400mg in 24 hours. Generally speaking, the first dose is taken 30 to 60 minutes before commencing travel.

Side effects Taking Gravol 50 (Dimenhydrinate) can cause sometimes cause:

  • Changes in behaviour
  • Loss of coordination
  • A ringing noise in the ears
  • Dryness affecting the mouth, throat or nose

If you experience side effects, it is recommended that you tell your doctor. In some cases, serious side effects may occur in patients taking this medicine. You must immediately seek medical attention if you notice any symptoms of a severe reaction.

Precautions Drinking alcoholic beverages while taking this medicine could worsen side effects, and you may be advised by your doctor to avoid drinking alcohol.

This medicine causes drowsiness. You should not drive if you become drowsy.

Seek immediate emergency medical attention if you suffer an allergic reaction. Symptoms to watch for, which may be indicative of a reaction, include skin rashes, hives, swelling of the face or limbs, trouble breathing, and trouble swallowing.

Always use Gravol 50 (Dimenhydrinate) as you have been prescribed by your doctor. Never self-medicate or change your dosage without first consulting your doctor. The correct dosage can vary depending on your health, medical history, and the severity of the condition being treated.

Travel sickness is caused by the repeated movements when travelling, the movement causes the inner ear to send differing signals to your brain from those your eyes are seeing. Hyoscine patches and antihistamine tablets have also proven to be very effective at preventing symptoms.

What is travel sickness and what causes it.

Travel sickness is also known as motion sickness . Travel sickness is a general term that incorporates seasickness, carsickness and airsickness depending on the mode of travel. The condition can cause the onset of several unpleasant symptoms, such as dizziness , nausea and vomiting . These symptoms are experienced when travelling. In the majority of cases the symptoms of travel sickness subside as the body adapts to the motions that cause the problem. Unfortunately, some sufferers are unable to adapt to the environment causing the condition so will continue to experience symptoms until their journey finishes or they leave said environment.

The vestibular system is a system in the body responsible for balance. It is comprised of a complex series of nerves, small channels and fluids that are located inside your inner ear. When the body is in motion, it causes the fluid inside the vestibular system to change position. This positional change is transmitted to the brain so that it is able to determine exactly how fast and where you are moving. This allows the body to maintain balance.

Motion sickness occurs when the body’s senses become confused . This confusion arises from conflicting information between the body’s other senses and the vestibular system. For example, if travelling in a car at forty miles an hour, your eyes register that you are in motion, travelling at speed. However, your vestibular system is telling your brain you are in fact sitting still. It’s this contradictory information that can lead to the symptoms of motion sickness.

In the majority of cases motion sickness is caused by travel, be that air, sea or land. However, motion sickness can sometimes occur from watching certain movies or playing fast-paced computer games. The shaking or motion of the camera work in some films, and the realistic graphics in computer games can generate the same mismatch of signals to the brain.

What are the symptoms of Travel Sickness?

The main symptom associated with travel sickness is nausea. This can be accompanied by feelings of discomfort in the upper abdomen as well a general sense of feeling unwell. This symptoms can then progress and can include;

  • Increased saliva production

In more severe cases symptoms can include; headaches, drowsiness, extreme tiredness and rapid, shallow breathing.

How is Travel Sickness diagnosed?

Diagnosing motion sickness will be result of observing the signs and symptoms and determining that they only occur when you are in motion . Most of the time motion sickness can be self-diagnosed without the need to see a doctor. If more severe symptoms occur, or if symptoms continue when motion has stopped, you should see a doctor for further investigation.

Can I do anything to prevent Travel Sickness?

There are a number of steps that can be taken to help minimise and relieve the symptoms of travel sickness. These include:

  • Trying to minimise head and body movements . If travelling by boat or plane then by choosing a cabin or seat situated in the middle of the craft, you will reduce the amount of movement suffered.
  • Getting fresh air . Where safe and possible open a window or move to the top deck of a ship to allow for a supply of fresh air.
  • Avoid eating large meals or drinking alcohol whilst travelling.
  • Fix your vision on a stable object . For example, fix your vision on the horizon.
  • Avoid reading and playing games as this can make the symptoms worse.
  • Closing your eyes may help to relieve symptoms.
  • Try to relax and stay calm . The more you worry about travel sickness the more likely you are to experience it.

What treatments are available for Travel Sickness?

There are various medicinal options available that can be used to treat travel sickness. These range from tablet and liquids to patches. If taking an oral medicine for motion sickness, it is recommended to take the medication before the journey . This is because travel sickness can slow normal digestion and therefore the body may not properly absorb medicine taken after the onset of symptoms. It may also be difficult to take medicine once nausea and sickness has set in.

Hyoscine is a widely used medicine used to treat travel sickness. It comes in both tablet form (Kwells & Joy Rides) and as an external patch ( Scopoderm ). Hyoscine works by blocking some of the nerve signals sent from the vestibular system to the brain. If using Kwells or Joy Rides the tablets will need to be taken before travel.

If embarking on a longer journey , Scopoderm patches will be a more suitable option. Scopoderm Patches can be applied to the skin and will continue to work for three days before the patch will need to be replaced. Scopoderm patches are suitable for use when nausea and sickness has already set in as they can be applied directly to the skin without the need to swallow medication.

Antihistamines can also be used to treat travel sickness as they are effective at controlling the symptoms of nausea and vomiting. There are several different types of antihistamine that are suitable in the treatment of motion sickness, these include:

  • Cinnarizine ( Stugeron Tablets )
  • Promethazine (Phenergan Tablets, Avomine Tablets )

Antihistamines are usually taken two hours before travelling. If it’s a long journey, the dose will need to be repeated after eight hours. You should check the individual dose for each product before taking any treatment.

Non-medicinal treatments are also available, for example acupressure bands (Sea-bands). These fabric bands are worn around the wrists and work by applying pressure to certain point that lie between the two tendons that run down your inner arm. This pressure is thought to help reduce the symptoms of travel sickness.

Alternatives

Non-Medicinal Alternatives for Travel Sickness

Acupressure Bands - are bands that are worn around the wrist. They are designed to apply pressure to a certain point on the inside of your wrist specifically between the two tendons on the inner arm. A positive for the acupressure bands is that there are no adverse side effects, how ever there is little scientific evidence to show that they are an effective treatment for motion sickness. You should be able to purchase this item from most pharmacies.

Travel Sickness FAQs

If you begin to experience mild travel sickness, there are a number of techniques that you can try which do not involve medicine.

Reading or looking at a screen will often make symptoms much worse. A common remedy is to close your eyes or otherwise, focus on a stationary point in the distance such as on the horizon. In a car, it might be helpful to sit in the front seat. Try to avoid sitting backwards on a train or boat.

Try to find a seat or position in which you will move as little as possible. On a boat or plane, this will be towards the middle of the vessel. It is especially important to keep your head still, which you can do with pillows or head rests.

It is important to remain calm. Try to find a technique that will help you to relax, such as concentrating on music or your breathing. You might also want to try mental activities which will distract you from your sickness, such as listing the alphabet backwards or making mental lists.

It may help your symptoms to get fresh air, either by opening windows or by moving outside or to drink water. This will help you to remain cool and hydrated.

Be mindful of what you eat and drink before and during any journey. Foods which are spicy, heavy or rich in fat can make travel sickness worse. Consider staying away from alcohol before or during travel if you know you are susceptible to travel sickness.

If you know that you are susceptible to travel sickness, you might want to take medication. This is most effective if taken prior to the journey so that symptoms do not develop in the first place. There are two primary types of travel sickness medication: hyoscine and antihistamines .

Hyoscine, or scopolamine, is available in a patch that can be purchased over the counter. Travel sickness occurs when the vestibular system in the inner ear senses imbalance or disequilibrium – hyoscine is thought to work by preventing those signals from affecting the brain.

Hyoscine’s side effects include dizziness, blurred vision, and drowsiness. Because of the nature of these side effects, you should not take hyoscine if you intend on driving. Children and the elderly should use hyoscine only with caution. It can have an adverse effect on people with a history of heart, liver or kidney issues, as well as those with conditions including epilepsy.

Antihistamines are most often used as a treatment for the symptoms of allergies. However, it can also prevent nausea, and so is sometimes effective as a remedy for travel sickness. Antihistamines tablets are taken one to two hours before a journey, or every eight hours during longer journeys. Unlike hyoscine, antihistamines will only help with the symptoms rather than the cause of nausea, and so it is a less effective treatment. However, apart from some drowsiness, there are usually fewer side effects.

These are two of several popular alternative remedies for travel sickness. The evidence that they are effective is inconclusive.

Acupressure bands are worn around your wrists. It is suggested that they work by applying pressure to a specific point on the wrist, which prevents the symptoms of travel sickness. There is little medical evidence that this is effective, though acupressure bands have no harmful side effects.

Ginger is widely used to treat nausea, including travel sickness. It is taken in the form of ginger drinks or biscuits, or as ginger supplements. Ensure with your GP that these supplements will not impact any of your other medication. Again, there is limited medical evidence of ginger as an effective travel sickness remedy.

Anyone can suffer from travel sickness, and indeed most people will experience it in some form. It is very common in young children, particularly those whose parents suffer from travel sickness. In many cases, however, children will grow out of it by the time they are teenagers.

Other groups that are vulnerable include pregnant women and people with certain medical conditions or on certain types of medication. This is especially true if nausea is already a symptom of your condition or a side effect of your medication.

The body will often adapt to the environment that is causing your symptoms, and so symptoms tend to improve over the course of a journey. For some people, however, the symptoms will continue until the journey is over.

Travel sickness is the body’s response to its current pattern of motion. Because this pattern is different for every mode transportation, certain journeys will likely make you iller than others. Experiences with travel sickness vary widely from person to person, and it is not possible to predict whether you will get sick on a particular journey. However, you can try to prevent or remedy travel sickness using the techniques mentioned above.

You are most likely to suffer from travel sickness when your body is experiencing an unfamiliar type of motion. So, as you become accustomed to a particular form of transport, it will often become more bearable. However, in some cases, the vestibular system will never adapt to a certain form of motion.

For some people, travel sickness occurs as the body’s default reaction to a certain form of motion. If this is the case, focus on relaxing and thinking positively.

Travel sickness can be frustrating and inconvenient, but it is not harmful to your health in the long term. In the vast majority of cases, symptoms go away once the journey is over.

There are a few cases in which symptoms will continue even after the journey is over. This is called Mal de Debarquement Syndrome – it occurs when the body does not properly readjust to being motionless on land.

Whilst travel sickness cannot be completely cured, its symptoms can be quite easily managed or prevented. Many people find non-medicinal remedies effective. These techniques are free of side effects, and so should be used before you consider taking additional medication.

Motion sickness (travel sickness) is common, especially in children. It is caused by repeated unusual movements during travelling, which send strong (sometimes confusing) signals to the balance and position sensors in the brain.

What is mal de debarquement syndrome?

Motion sickness is a normal response to repeated movements, such as going over bumps or around in a circle, send lots of messages to your brain. If you are inside a vehicle, particularly if you are focused on things that are inside the vehicle with you then the signals that your eyes send to the brain may tell it that your position is not changing, whilst your balance mechanisms say otherwise.

Your balance mechanisms in your inner ears sense different signals to those that your eyes are seeing which then sends your brain mixed, confusing messages. This confusion between messages then causes people to experience motion sickness.

Is motion sickness normal?

Motion sickness is a normal response that anyone can have when experiencing real or perceived motion. Although all people can develop motion sickness if exposed to sufficiently intense motion, some people are rarely affected while other people are more susceptible and have to deal with motion sickness very often.

Triggers for motion sickness

Motion sickness can also be triggered by anxiety or strong smells, such as food or petrol. Sometimes trying to read a book or a map can trigger motion sickness. Both in children and adults, playing computer games can sometimes cause motion sickness to occur.

Motion sickness is more common in children and also in women. Fortunately, many children grow out of having motion sickness. It is not known why some people develop motion sickness more than others. Symptoms can develop in cars, trains, planes and boats and on amusement park rides, etc.

Symptoms typically go when the journey is over; however, not always. In some people they last a few hours, or even days, after the journey ends.

There are various symptoms of motion sickness including::

  • Feeling sick (nausea and vomiting).
  • Sweating and cold sweats.
  • Increase in saliva.
  • Headaches .
  • Feeling cold and going pale.
  • Feeling weak.

Some general tips to avoid motion sickness include the following.

Prepare for your journey

  • Don't eat a heavy meal before travelling. Light, carbohydrate-based food like cereals an hour or two before you travel is best.
  • On long journeys, try breaking the journey to have some fresh air, drink some cold water and, if possible, take a short walk.

Plan where you sit

  • Keep motion to a minimum. For example, sit in the front seat of a car, over the wing of a plane, or on deck in the middle of a boat.
  • On a boat, stay on deck and avoid the cafeteria or sitting where your can smell the engines.

Breathe fresh air

  • Breathe fresh air if possible. For example, open a car window.
  • Avoid strong smells, particularly petrol and diesel fumes. This may mean closing the window and turning on the air conditioning, or avoiding the engine area in a boat.

Use your eyes and ears differently

  • Close your eyes (and keep them closed for the whole journey). This reduces 'positional' signals from your eyes to your brain and reduces the confusion.
  • Don't try to read.
  • Try listening to an audio book with your eyes closed. There is some evidence that distracting your brain with audio signals can reduce your sensitivity to the motion signals.
  • Try to sleep - this works mainly because your eyes are closed, but it is possible that your brain is able to ignore some motion signals when you are asleep.
  • Do not read or watch a film.
  • It is advisable not to watch moving objects such as waves or other cars. Don't look at things your brain expects to stay still, like a book inside the car. Instead, look ahead, a little above the horizon, at a fixed place.
  • If you are the driver you are less likely to feel motion sickness. This is probably because you are constantly focused on the road ahead and attuned to the movements that you expect the vehicle to make. If you are not, or can't be, the driver, sitting in the front and watching what the driver is watching can be helpful.

Treat your tummy gently

  • Avoid heavy meals and do not drink alcohol before and during travelling. It may also be worth avoiding spicy or fatty food.
  • Try to 'tame your tummy' with sips of a cold water or a sweet, fizzy drink. Cola or ginger ale are recommended.

Try alternative treatments

  • Sea-Bands® are acupressure bands that you wear on your wrists to put pressure on acupressure points that Chinese medicine suggests affects motion sickness. Some people find that they are effective.
  • Homeopathic medicines seem to help some people, and will not make you drowsy. The usual homeopathic remedy is called 'nux vom'. Follow the instructions on the packet.

All the techniques above which aim to prevent motion sickness will also help reduce it once it has begun. Other techniques, which are useful on their own to treat motion sickness but can also be used with medicines if required, are:

  • Breathe deeply and slowly and, while focusing on your breathing, listening to music. This has been proved to be effective in clinical trials.
  • Ginger - can improve motion sickness in some people (as a biscuit or sweet, or in a drink).

There are several motion sickness medicines available which can reduce, or prevent, symptoms of motion sickness. You can buy them from pharmacies or, in some cases, get them on prescription. They work by interfering with the nerve signals described above.

Medicines are best taken before the journey. They may still help even if you take them after symptoms have begun, although once you feel sick you won't absorb medicines from the stomach very well. So, at this point, tablets that you put against your gums, or skin patches, are more likely to be effective.

Hyoscine is usually the most effective medicine for motion sickness . It is also known as scopolamine. It works by preventing the confusing nerve messages going to your brain.

There are several brands of medicines which contain hyoscine - they also come in a soluble form for children. You should take a dose 30-60 minutes before a journey; the effect can last up to 72 hours. Hyoscine comes as a patch for people aged 10 years or over. (This is only available on prescription - see below.) Side-effects of hyoscine include dry mouth , drowsiness and blurred vision.

Side-effects of motion sickness medicines

Some medicines used for motion sickness may cause drowsiness. Some people are extremely sensitive to this and may find that they are so drowsy that they can't function properly at all. For others the effects may be milder but can still impair your reactions and alertness. It is therefore advisable not to drive and not to operate heavy machinery if you have taken them. In addition, some medicines may interfere with alcohol or other medication; your doctor or the pharmacist can advise you about this.

Antihistamines

Antihistamines can also be useful , although they are not quite as effective as hyoscine. However, they usually cause fewer side-effects. Several types of antihistamine are sold for motion sickness. All can cause drowsiness, although some are more prone to cause it than others; for example, promethazine , which may be of use for young children on long journeys, particularly tends to cause drowsiness. Older children or adults may prefer one that is less likely to cause drowsiness - for example, cinnarizine or cyclizine.

Remember, if you give children medicines which cause drowsiness they can sometimes be irritable when the medicines wear off.

There are a number of anti-sickness medicines which can only be prescribed by your doctor. Not all of them always work well for motion sickness, and finding something that works may be a case of trial and error. All of them work best taken up to an hour before your journey, and work less well if used when you already feel sick. Hyoscine patch

Hyoscine, or scopolamine, patches are suitable for adults and for children over 10 years old. The medicine is absorbed through your skin, although this method of medicine delivery is slow so the patch works best if applied well before your journey.

You should stick the patch on to the skin behind the ear 5-6 hours before travelling (often this will mean late on the previous night) and remove it at the end of the journey.

Prochlorperazine

Prochlorperazine is a prescription-only medicine which works by changing the actions of the chemicals that control the tendency to be sick (vomit), in your brain. One form of prochlorperazine is Buccastem®, which is absorbed through your gums and does not need to be swallowed. Buccastem® tastes rather bitter but it can be effective for sickness when you are already feeling sick, as it doesn't have to be absorbed by the stomach.

Metoclopramide

Metoclopramide is a tablet used to speed up the emptying of your tummy. Slow emptying of the tummy is something that happens when you develop nausea and vomiting, so metoclopramide can help prevent this. It prevents nausea and vomiting quite effectively in some people. It can occasionally have unpleasant side-effects, particularly in children (in whom it is not recommended). Metoclopramide is often helpful for those who tend to have gastric reflux, those who have slow tummy emptying because of previous surgery, and those who have type 1 diabetes. Your GP will advise whether metoclopramide is suitable for you.

Domperidone

Domperidone , like metoclopramide, is sometimes used for sickness caused by slow tummy emptying. It is not usually recommended for motion sickness but is occasionally used if other treatments don't help. Domperidone is not a legal medicine in some countries, including the USA.

Ondansetron

Ondansetron is a powerful antisickness medicine which is most commonly used for sickness caused by chemotherapy, and occasionally used for morning sickness in pregnancy. It is not usually effective for motion sickness. This, and its relatively high cost means that it is not prescribed for motion sickness alone. However, for those undergoing chemotherapy, and for those who have morning sickness aggravated by travel, ondansetron may be helpful.

What should I do if I'm actually sick?

If you're actually sick you may find that this relieves your symptoms a little, although not always for very long. If you've been sick:

  • Try a cool flannel on your forehead, try to get fresh air on your face and do your best to find a way to rinse your mouth to get rid of the taste.
  • Don't drink anything for ten to twenty minutes (or it may come straight back), although (very) tiny sips of very cold water, coke or ginger ale may help.
  • After this, go back to taking all the prevention measures above.
  • Once you reach your destination you may continue to feel unwell. Sleep if you can, sip cold iced water, and - when you feel ready - try some small carbohydrate snacks. Avoid watching TV (more moving objects to watch!) until you feel a little better.

The sensation called 'mal de debarquement' (French for sickness on disembarking) refers to the sensation you sometimes get after travel on a boat, train or plane, when you feel for a while as though the ground is rocking beneath your feet. It is probably caused by the overstimulation of the balance organs during your journey. It usually lasts only an hour or two, but in some people it can last for several days, particularly after a long sea journey. It does not usually require any treatment.

Persistent mal de debarquement syndrome is an uncommon condition in which these symptoms may persist for months or years.

For prevention and treatment of these symptoms associated with motion sickness:

for children under 2 years of age unless directed by a doctor.

Ask a doctor before use if you have

  • a breathing problem such as emphysema or chronic bronchitis
  • trouble urinating due to an enlarged prostate gland

Ask a doctor or pharmacist before use if you are taking

sedatives or tranquilizers

When using this product

  • marked drowsiness may occur
  • avoid alcoholic drinks
  • alcohol, sedatives, and tranquilizers may increase dizziness
  • be careful when driving a motor vehicle or operating machinery or any activity that requires alertness

If pregnant or breast-feeding,

ask a doctor before use.

Keep out of reach of children.

In case of accidental overdose, get medical help or contact a Poison Control Center (1-800-222-1222) right away.

  • to prevent motion sickness, the first dose should be taken ½ to 1 hour before starting activity
  • to prevent or treat motion sickness, see below:

What is dimenhydrinate?

Dimenhydrinate is for use in adults and children at least 2 years old to treat or prevent nausea , vomiting and dizziness caused by motion sickness .

Dimenhydrinate may also be used for purposes not listed in this medication guide.

Dimenhydrinate side effects

Get emergency medical help if you have signs of an allergic reaction : hives, difficult breathing, swelling of your face, lips, tongue, or throat.

Dimenhydrinate may cause serious side effects. Call your doctor at once if you have:

severe drowsiness ;

wheezing , trouble breathing, severe or worsening cough ;

blurred vision, eye pain or redness, seeing halos around lights; or

painful or difficult urination.

Less serious side effects may be more likely, or you may have none at all.

This is not a complete list of side effects and others may occur. Call your doctor for medical advice about side effects. You may report side effects to FDA at 1-800-FDA-1088.

Before taking this medicine

You should not use dimenhydrinate if you are allergic to it.

Ask a doctor or pharmacist if dimenhydrinate is safe to use if you have or have ever had:

an enlarged prostate or urination problems;

breathing problems such as emphysema , or chronic bronchitis ;

glaucoma ; or

if you recently used alcohol, sedatives, or tranquilizers.

The chewable tablet of dimenhydrinate may contain phenylalanine. Tell your doctor before taking if you have phenylketonuria (PKU).

Ask a doctor before using this medicine if you are pregnant or breastfeeding.

Do not give this medicine to a child without medical advice.

How should I take dimenhydrinate?

Use exactly as directed on the label, or as prescribed by your doctor. Do not use this medicine in larger amounts or more often than directed.

For best results, take dimenhydrinate 30 to 60 minutes before traveling or before any activity that may trigger motion sickness.

You must chew the chewable tablet before you swallow it.

Store at room temperature away from moisture and heat.

What happens if I miss a dose?

Dimenhydrinate is used when needed. If you are on a dosing schedule, skip any missed dose. Do not use two doses at one time.

What happens if I overdose?

Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.

What should I avoid while taking dimenhydrinate?

Avoid driving or hazardous activity until you know how dimenhydrinate will affect you. Dizziness or drowsiness can cause falls, accidents, or severe injuries.

Avoid drinking alcohol. Drinking alcohol can increase certain side effects of dimenhydrinate.

What other drugs will affect dimenhydrinate?

Using dimenhydrinate with other drugs that make you drowsy can worsen this effect. Ask your doctor before using opioid medication, a sleeping pill, a muscle relaxer, sedative, tranquilizer, or medicine for anxiety or seizures .

Other drugs may affect dimenhydrinate, including prescription and over-the-counter medicines, vitamins , and herbal products . Tell your doctor about all other medicines you use.
