As the situation remains very fluid, the relevant Government and/or Health Authorities may change protocols, at any given time, or in particular to any given passenger or case.

Flying to Zanzibar:

All passengers travelling from South Africa to Zanzibar, irrespective of airline must be in possession of a negative PCR test not older than 72 hours prior to travel. All passengers will be required to complete Port Health Exit Screening Questionnaire digitally at least 2 days prior to travel ( https://sa-covid-19-travel.info/exitq/create ) as well as Zanzibar’s Traveller’s Surveillance Form for the flight out of South Africa to Zanzibar, and will be subject to temperature and advanced screening at both airports.

Flying back to South Africa:

All passengers flying back into oR Tambo from Zanzibar will be required to produce a negative PCR test in order to board the flight.

Additionally, all passengers will be required to complete Zanzibar’s Surveillance Form for exit.

South Africa’s Passenger Health Questionnaire for re-entry, which is available digitally ( https://sa-covid-19-travel.info/entryq/create ), must be completed no later than 2 days prior to travel.

We can assist with the PCR test in Zanzibar: The cost is R1650 per person, including the PCR test fee and the medical representative (prepayable prior to your departure). At the time of booking we will require passport copies and a valid email address to input into the PCR registration portal. Our associate office in Zanzibar will collect you from your resort and escort you to a testing facility in Stone Town and will return your test certificate to you prior to your return flight to South Africa. This paperwork, indicating a negative test result, must be presented to Port Authority on arrival in South Africa. Please note that authorities in Zanzibar do not work on weekends or public holidays and kit availability is limited so should you not be able to test in Zanzibar for reasons beyond our control your prepayment will be refunded on our first working day.

All costs associated to quarantining in Zanzibar (in the event of a positive Covid test result) shall be for the passengers’ account.

Children under the age of 5 are exempt from PCR testing.

We recommend pre-printing and completing the Zanzibar Surveillance Form as well as South African Passenger Health Questionnaire. Own sanitiser may be travelled with, up to 100ml per person, in a container that does not exceed this limitation requirement.

Masks are to be worn at all times when at both airports and in flight. It is suggested that you pack multiple masks for use, as even though masks are not considered mandatory for Zanzibar excursions, it is encouraged.

Please note that it remains the relevant country’s health authority’s prerogative to change quarantine and/or COVID protocols, at any given time. Please note that many airlines offer little, to no, onboard food service so we recommend that you take any snacks or refreshments for the duration of your flight journey with you.

ENTRY REQUIREMENTS

All travellers whether foreign nationals, Tanzanian citizens or Residents, MUST enter Tanzania through designated (formal) Entry Points. Upon arrival, the traveller will be responsible for:

  • Appearing in person in front of an Immigration Officer with a completed and signed arrival declaration card, and
  • Producing their passport or other acceptable document to an Immigration Officer; and
  • In the case of foreign nationals; they may be required:
  • To show their Visa or apply for Visa on arrival;
  • To show Residence Permit/Pass If they are bonafide Residents in Tanzania;
  • Return ticket or evidence of onward travel arrangement; and
  • Evidence of funds for maintenance (upkeep budget

Tanzania Health Form

The Tanzanian authorities introduced a short health declaration for all incoming travelers to Tanzania during the ongoing Covid-19 pandemic. The form is mandatory to be completed before arriving in Tanzania and is available entirely online.

Completing the Tanzanian Public Health Surveillance Form takes only a couple of minutes and can be done using any Internet-accessed electronic device.

Many countries worldwide introduced health declarations as an additional safety measure to tackle the coronavirus pandemic and control the spread of new cases. The health declaration then ensures the safety of both incoming visitors and Tanzanian citizens. Since travelers are required to provide some essential personal, travel, and health details, the Tanzanian officials are able to immediately contact a traveler who may have been infected or contacted an infected person.

Every applicant who successfully submits the Tanzanian Health Form will receive its confirmation to the email address provided when filling out the online form.

Who must complete the Tanzania Health Form?

The Tanzania Health Surveillance Form is mandatory for everyone arriving in Tanzania. Regardless of age, nationality, and vaccination status, every person needs to complete the health form before the planned trip.

Children traveling to Tanzania should complete a separate health form for Tanzania. Nevertheless, their legal guardians or parents need to make the application on their behalf. They also must ensure that all the data of their underage travel companion is correct and up-to-date.

When should you submit the Tanzania Health Form application?

You must submit your Tanzania Health Surveillance Form application within 24 hours before the planned arrival. It is extremely important not to complete the declaration earlier since all the data provided must be up-to-date. Filling out the online declaration before this specific time frame may result in some problematic issues upon crossing the Tanzanian borders or even denying entry.

The Tanzanian authorities introduced the requirement of a specific time to complete the form to ensure the safety of all incoming people and Tanzanian nationals.

The Tanzania Health Surveillance Form - Application Instruction

To successfully complete the Tanzania Health Surveillance Form, every applicant should follow these simple steps:

  • First of all, prepare the above-listed details and go to the online application form.
  • Next, provide all the necessary details in the online declaration.
  • Double-check the entered information.
  • Pay the processing fee by choosing one of the available methods of online payment.
  • Check whether the confirmation email has already been delivered to your inbox.

The confirmation will be sent as a PDF file. All travelers will need to print it out and carry it for the trip to Tanzania. Make sure you have the copy with you! The border officials may ask you to show the confirmation during the border control.

Other requirements to enter Tanzania during Covid-19

Apart from the necessity to complete the Tanzania Health Surveillance Form, all travelers to Tanzania need to submit a negative report of the Covid-19 test taken within 96 hours before the scheduled departure. The Tanzanian authorities recognize both RT-PCR and NAATs.

Travelers who arrive from high-risk countries, where an increased number of coronavirus cases have been reported, must additionally be tested upon arrival at their own cost. If tested positive for Covid-19, they are subject to a 14-day quarantine that may be ended only upon receiving the negative test result.

If you plan to visit also Zanzibar, you should know that Zanzibar requires a separate health declaration dedicated only to the island.

Tanzania has opened up and relaxed COVID-19 regulations for tourists. Visitors no longer need a Health Surveillance form for mainland Tanzania. However, those planning to go to Zanzibar will still need to fill out the Tanzania Health Surveillance form.

The Health Surveillance Form will no longer be used by Tanzania as they reduce COVID-19 travel restrictions. However, the Revolutionary Government of Zanzibar revealed that it will continue to reinforce the implementation of COVID-19 mitigation measures to ensure the safety of everyone on the island.

With this in mind, registering to the health surveillance platform and filling out the form will remain mandatory for all visitors coming to the island. Here is everything you need to know about the Tanzanian health surveillance form and how to apply with iVisa .

What is the Tanzania Health Surveillance Form?

The Tanzania Traveler Health Surveillance is a screening document to monitor the health of travelers entering Zanzibar only. The form is part of Tanzania’s and Zanzibar’s health surveillance systems implemented during the COVID-19 pandemic. While you don’t need this form anymore to visit mainland Tanzania, it is still mandatory to submit it if you’re planning to go to Zanzibar.

Please note that it does not replace a visa. If your nationality requires a visa to enter Tanzania and Zanzibar, you will be required to present both the visa and the form upon arrival.

Who requires a Health Surveillance Form to enter Zanzibar?

All travelers, including tourists, children, returning residents, and crew members entering Zanzibar , both by air or sea, must fill out the Public Health Surveillance Form online. The form must be filled out by each member of a travel group.

How long is the Zanzibar Health Form valid for?

The form must be completed online at least 24 hours before your arrival to Zanzibar and is valid for a single entry and a stay of 30 days.

Requirements and documents needed for the Zanzibar Health Form

Although there are no document requirements for the Traveler's Surveillance Form, we will need you to provide the following information:

Your passport information , such as your passport number and expiry date.

Details of your trip including flight/boat times and dates.

Do I need a Health Surveillance Form if I’m transiting through Zanzibar?

Yes, all passengers transiting through Zanzibar are required to fill out a Tanzania Health Surveillance Form.

Do minors need to fill out their own Health Form to visit Zanzibar?

Minors and children are also required to present a Tanzania Health Form of their own . The guardian or parent traveling with the minor is required to fill out the form on their behalf.

There are three possible visas that you can apply for to enter Tanzania and Zanzibar depending on your intentions to enter the country. They are:

Tanzania Tourist eVisa : The Tanzania e-visa allows citizens from eligible countries to travel to Tanzania and Zanzibar for tourism and leisure purposes, such as visiting family or friends.

Tanzania Transit Visa : A transit visa allows you to cross the country from one point of entry to another destination, i.e. enter Tanzania and simply pass through without exploring the country.

Tanzania Volunteer eVisa : In order to obtain a Tanzania Volunteer e-Visa, applicants must have a volunteering contract between them and the organization they will be volunteering for to prove their intent to volunteer. You may also need an invitation letter that confirms the purpose of the trip.

What else should I keep in mind before traveling to Tanzania/Zanzibar?

We recommend that all travelers to the island check updated information regarding COVID-19 protocols currently in place via government or airline sites, as these tend to change quickly.

Once you’ve checked the travel restrictions, got your visa sorted, and packed your bags, remember to apply for your Tanzania Health Surveillance form 24 hours before your flight.
